Subhadra is worshiped along with her brothers Krishna (the avatar of Vishnu) and Balarama (the avatar of Shesha-naga).

As such Was she also an avatar of any Goddess?

If so, Which Goddess and what is the source of that information?

There seems to be no clear reference. Some interpretations of Harivamsha say that she was incarnation of Ekanamsha, a rupa of YogaNidraa, however, I think that is a misinterpretation. The daughter that was born of Devaki BEFORE Krishna's birth was YogaNidraa. That was it would still be correct to call that incarnation a sister of Krishna, but need not necessarily have to point to Subhadra.

The Puri Jagannath-dhaam lore considers her thus: "Subhadra is adored as the embodiment of His enchanting delusion" (ref: http://magazines.odisha.gov.in/Orissareview/sept-oct2007/engpdf/Pages26-29.pdf).

Probably that's all you will get.
